The further you are, the images break

Got a client reporting that images are breaking.

I checked my logs and we saw

client denied by server configuration

All right what could be the culprit?

You get this when "Order allow, deny" and then "Deny..." something.
Or when you "Order deny, allow" and then don't "Allow..." enough.
